Martin's Point is pleased to offer this new online service to current members of the US Family Health Plan. Using this online portal, you can easily:

  • Request a replacement ID Card.
  • Add a new primary care provider (for new members) or change to a different Primary Care Provider.  You'll be able to select from our network of providers.
  • View and update your personal information.
  • Send a secure message to Member Services.
  • Take the Health Survey.
First-time visitors:
To access your account information, you must first register by clicking Register at the top of the page.  You will need to create a username and password and provide your name, date of birth, and member ID number. Be sure to remember your username and password for the next time you login.
